Treatment History

Clinical Diagnostics

Record and manage diagnoses and clinical problems in a patient's Dentaltap treatment history.

Open Diagnostics

  1. Open a patient.
  2. Select Diagnostics in the clinical history tabs.
  3. Review the existing records before adding a new one.

The section can separate unresolved and resolved records. Use the status controls to focus on either group.

Create Diagnostic Record

  1. Click Add Record.
  2. Choose whether you are adding a diagnosis or a problem when the form offers that choice.
  3. Search for the appropriate code or description.
  4. Add priority, teeth, or areas when relevant.
  5. Review the record and save it.

The available fields and codes depend on the clinic's configured dictionaries and your permissions.

Review and manage records

A diagnostic record can show:

  • diagnosis or problem name;
  • code, including an ICD code when available;
  • priority;
  • selected teeth or areas;
  • resolution status;
  • history of changes.

Use the record actions to open or manage the item when those actions are available.

Update Diagnostic Record

Use the record actions to correct the diagnosis details, change the affected teeth or area, adjust the priority, or move the record between active and resolved states.

Delete Diagnostic Record

Delete a diagnostic record only when it was added by mistake or should not remain on the chart. Dentaltap also supports bulk deletion when several records were entered incorrectly.

Clinical accuracy

Choose the most specific configured diagnosis or problem that matches the clinical information. Check the selected teeth, priority, and resolution state before saving.
