Clinical Notes

Record patient-level or appointment-related clinical notes and structured exams.

Open Clinical Notes

  1. Open a patient.
  2. Select Clinical Notes in the clinical history tabs.
  3. Choose the correct appointment context when the note belongs to a particular visit.

The default view shows All Notes. If clinical exams are attached, each exam can appear as its own tab.

Create Clinical Note

  1. Enter the note in the clinical note editor.
  2. Attach files when supporting material is required.
  3. Review the text and attachments.
  4. Save the note.

When an appointment is selected, the note is linked to that visit. Use All History for information that belongs to the patient rather than to one appointment.

Add a structured exam

  1. Click Add Exam.
  2. Choose an exam template configured by your clinic.
  3. Complete the structured fields, questions, comments, and tooth information when provided.
  4. Save the exam.

After an exam is attached, select its exam tab to review it. Use Edit Exam to update it when permitted, and use the print action when a printable exam is available.

Manage notes

Depending on your role, note actions can include:

  • Pin or Unpin an important note;
  • Lock or Unlock a note;
  • Edit a note;
  • Delete a note;
  • open an attached file.

Locked notes cannot be edited or deleted until they are unlocked by an authorized user.

Update Clinical Note

Open the saved note to update its text, attachments, or status when your role allows it, then save the changes.

Delete Clinical Note

Delete a clinical note only when it should be fully removed from the patient record. If the note is locked, it may need to be unlocked first by an authorized user.

Write useful clinical notes

Keep each note specific, factual, and connected to the correct patient and visit. Use Clinical Notes for care-related information and the appointment note for scheduling or operational instructions.
