Questionnaires

Review patient questionnaire answers in Dentaltap before treatment and use them to prepare for the visit.

Open questionnaires

  1. Open Patients and select the patient.
  2. In the clinical history column, open Overview.
  3. Open the Questionnaires card.

Depending on its status, you may see a new form, submitted answers, a clinical summary, or a history of previous questionnaires with their dates and statuses.

Create Questionnaire

To create a questionnaire response, complete the form for the patient and check the following before saving:

  • confirm that it belongs to the correct patient;
  • complete the questions carefully;
  • review the summary before saving;
  • make sure a fresh response is captured when the clinic needs one for the current visit.

Select Save Changes when you are ready to add the response to the patient's record.

Request the questionnaire from the patient

If the patient should complete the form, select Request Questionnaire. Dentaltap opens the patient portal flow, allowing the patient to respond on their own device instead of using the clinic tablet.

When the questionnaire already belongs to the current appointment, the same area can open the saved questionnaire in the patient portal instead of creating a new request.

Review alert suggestions after submission

Questionnaire answers are connected to the medical-alert workflow. After a response is saved, Dentaltap can suggest medical alerts from the answers and add them to the patient's record as unconfirmed items.

Review Medical Alerts after an important questionnaire update, then confirm the suggested alerts that belong in the patient's active record.

Review previous responses

Questionnaire history helps you confirm whether:

  • the patient already submitted a form;
  • the answers are still current;
  • a new response is needed before treatment;
  • the clinic needs to review newly suggested alerts.

Update Questionnaire

Open the saved questionnaire response or its details view, adjust the answers that changed, and click Save Changes.

Delete Questionnaire

Delete a questionnaire only when it was recorded by mistake or belongs to the wrong patient. After deletion, review Medical Alerts if that questionnaire created alert suggestions.
