Consent Forms

Where consent is usually handled
Consent is often collected through the same patient form workflow used for questionnaires and documents. The exact entry point depends on the clinic setup, but the goal is the same: record the patient's acknowledgement and keep the signed form in the patient record.
Create Consent Form
Open the document flow from the patient record, choose the configured consent form, confirm the patient and visit context, and save the form before requesting signatures.
This keeps the consent attached to the right patient record from the beginning.
Collect signatures the right way
Clinic users can sign consent-related forms directly in Dentaltap. The signature history records who signed and when, allowing the team to verify the form later.
If the patient needs to sign, send the patient-facing form through the patient portal so they can sign on their own device. This avoids handing over a clinic device with Dentaltap open.
This is especially useful when:
- the patient is at reception or away from the chair;
- the doctor wants to request a signature before treatment starts;
- another clinic user needs to review and sign from their own account.
Review consent status
Before treatment continues, confirm that the required consent is:
- attached to the correct patient and form;
- already signed or still pending;
- signed by the right person when the clinic workflow requires it;
- still current for the procedure or visit.
If a signature is missing, request it through the patient portal or complete the clinic-side signature step before proceeding.
Update Consent Form
Open the saved consent document to correct patient-specific details, update the linked context, or refresh the form before signatures are finalized.
Delete Consent Form
Delete a consent form only when it was created in error, attached to the wrong context, or should be fully removed under the clinic's record policy.
