Create Patient

Create a new patient record in Dentaltap and enter the identity and contact details needed for care.

Open Patients from the left navigation.

Create a patient

  1. Click Create Patient.
  2. Enter the patient's Full Name.
  3. Complete the information that your clinic needs, such as gender, birthday, MRN, phone, email, language, and patient groups.
  4. Use the section list to move between Overview, Assign, Contacts, Identity, Address, Emergency, Primary Care, Additional, and Family when those sections are enabled for your clinic.
  5. Review the form before saving.
  6. Click Create Patient.

The new record appears in the patient list. Depending on the clinic configuration, the medical record number (MRN) may be assigned automatically.

Important profile sections

  • Overview contains the patient's name, gender, birthday, age, and MRN.
  • Assign connects the patient with the clinic's groups, branch, or responsible users when configured.
  • Contacts stores phone numbers, email, language, messaging preferences, and optional guarantor information.
  • Identity stores identity documents or other configured identity values.
  • Address stores street, city, province, and postal information.
  • Emergency stores the emergency contact and phone number.
  • Primary Care stores the external doctor or clinic details.
  • Additional stores company, position, and clinic-specific custom fields.
  • Family connects related patients and family members.

The available sections and custom fields depend on your clinic configuration. Complete every required field before saving the patient.

Common questions

I cannot create another patient

Your clinic may have reached its patient limit or your role may not have permission to create patients. Contact a clinic administrator if the Create Patient action is unavailable.
